When will a nurse
or therapist see me?
Frequency of nursing or therapist visits may range from daily to monthly visits, with this coordinated through your physician. Services will be provided during regular office hours,
7:30 a.m. to 4:00 p.m., Monday through Friday. For rare incidences services may be offered by the nursing staff on weekends and holidays.
How do I know whether I or one of my family members qualify for Home Health Care?
There are three simple patient criteria:
You must be under the care of a physician, having seen your physician within the last 60 days.
You must require skilled nursing care or therapy which requires the expertise of a nurse or physical therapist.
You must be homebound, only able to leave home for medical necessity or on infrequent occasions.
Who pays for all these services?
St. Luke Home Health Agency accepts Medicare,
Medicaid, Worker's compensation, and private pay health
insurance.
What are the Home Health office hours?
Regular office hours are 7:30 a.m. to 4:00 p.m., Monday through Friday. The On-Call Home Health Nurse may be reached by calling our Answering Service at 1-800-618-0144. Please remember the Home Health Agency is not an emergency agency, therefore, conditions of an emergency nature should be directed to the SMRMC Emergency Room for evaluation.
How do I get Home Health care?
Ask your physician during a visit, hospitalization or by phone. He or she will then notify the Home Health office about your care. You also may contact the St. Luke Home Health office and the nurse will contact your doctor for you.
