Southwest Mississippi Regional Medical Center maintains medical records for each patient treated at the hospital.

To request a copy of your medical records, please contact our Health Information Management/Release of Information Department at 601-250-4453 between the hours of 8:00 am to 5:00 pm, Monday through Friday.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid photo ID.

Records needed for continuing care purposes will be send directly to your provider.

Requests for medical records generally take up to 72 hours to process.
